Instrument Availability
The LODHI-A (Longitudinally Oriented Diagnostic Historical Interview — ADHD) is a clinical instrument developed by Shafi Lodhi, MD. It is available to licensed clinicians on request through the access form on the homepage, and is delivered by email.
Intended Users
The LODHI-A is designed for licensed mental health professionals trained in adult psychopathology and differential diagnosis: psychiatrists, primary care physicians, psychologists, psychiatric nurse practitioners, physician assistants, licensed clinical social workers (LCSWs), licensed professional counselors (LPCs), licensed mental health counselors (LMHCs), licensed professional clinical counselors (LPCCs), and other clinicians trained in diagnostic assessment, practicing within their scope. Access is provided on a self-attestation basis, consistent with how comparable clinical interviews are distributed. The LODHI-A is not a self-assessment tool and is not appropriate for use by unlicensed practitioners, life coaches, ADHD coaches, or peer counselors.
Terms of Use
Licensed clinicians may use the LODHI-A in their clinical practice without fee, including in private practice, institutional settings, and telemedicine. Reproduction for distribution, modification of the instrument, redistribution, incorporation into institutional protocols, or commercial use requires prior written permission from the author. Use of the LODHI-A in published research requires a separate research agreement, available upon request.
Licensing Philosophy
The LODHI-A is offered to licensed clinicians at no cost because a well-designed diagnostic tool should be available to every practitioner who can use it responsibly, not restricted by prohibitive pricing or institutional gatekeeping. Permission requirements for redistribution and modification exist to protect the integrity of the methodology, not to generate revenue from clinical use.
What Is Included
Access to the LODHI-A includes the full Clinical Interview Guide with all nine modules, clinical notes, and appendices; a chairside Question Sheet containing just the interview prompts; the behavioral observation protocol; the running differential framework and tracking sheet; the LODHI-A-COL collateral informant forms in both partner and parent/family versions; the telemedicine adaptation guide; and the clinical manual with module-by-module guidance, cultural considerations, and administration best practices.
